You'll work directly with a small team.
Not a phone tree. The care coordinator who handles your intake is the same one who'll stay with you through the episode.
About 76 Health
The healthcare system was built in pieces. We're bringing them together.
The calls. The referrals. The paperwork. The uncertainty. The waiting.
Those moments in between visits determine whether people actually get better.
We're building a model that helps close those gaps. That brings care together around the person receiving it. Because nobody should have to navigate recovery alone.
What we believe

Right now, we're working with a small number of patients as we build and refine a better way to navigate recovery. If you start with us now, a few things are true:
Not a phone tree. The care coordinator who handles your intake is the same one who'll stay with you through the episode.
Small caseloads mean more time on each person: calls returned the same day, plans reviewed weekly, and adjustments made when something isn't working.
Early patients tell us what's working and what isn't, and we change accordingly. If something feels off, we want to hear it.
If your case isn't a fit for our current model, we'll tell you on the intake call and help you find the right place. We'd rather lose a patient at the front door than fail to serve them well.
Looking ahead
Because nobody should have to go through recovery alone.
Not patients. Not the clinicians who care for them. Not the practices holding it all together.
Spend more time with patients, less on logistics. We work alongside providers so they can focus on delivering care.
