Providers

Focus on care.
Leave the rest to us.

You didn't get into healthcare to spend your day dealing with paperwork. Tell us a little about your practice, and we'll be in touch within two business days.

What happens next

  1. 01We learn more about your practice and what you’re looking for.
  2. 02We schedule a conversation to see if there’s a fit on both sides.
  3. 03We’ll walk you through the partnership options and answer any questions.

Onboarding typically takes two to eight weeks, depending on the type of partnership.

Step 1 of 3

Tell us about you.

Let's start with the basics.

What partnership looks like

Practice medicine. Not paperwork.

01.

Patients ready to begin care.

Patients arrive with the groundwork already done: intake completed, insurance verified, and a care plan already in motion. You spend less time on logistics and more on care.

02.

Support that fits your practice.

Some partners want coordinated patient flow. Others want help with billing, credentialing, compliance, staffing, and operations. Choose what works for you.

03.

Outcomes infrastructure built in.

Track patient progress with validated outcome measures and clear reporting. Over time, you'll build a record of the results your practice delivers.

04.

Predictable, transparent compensation.

Clear agreements. Transparent payment structures. Reliable biweekly payments. No surprises.

05.

Physician-led, evidence-based care plans.

Every patient comes with a physician-reviewed plan, clear goals, and measurable milestones. All while you retain full clinical autonomy.

06.

No software to install. No EHR to change.

You keep your EHR. You keep your billing system. You keep your scheduling. We do our best to integrate around you.

A practice owner reviewing their panel

The owners we're built for.

Launch your own practice with support.

For providers ready to go out on their own, we provide the infrastructure, operations, and patient panel needed to get started with confidence.

Stay independent without doing it alone.

Keep control of your practice while gaining the support, infrastructure, and partnership you need.

Run a practice that proves its outcomes.

We measure with the same instruments researchers and academic centers use. The data is yours.

Think about the next chapter on your own terms.

Whether you're focused on growth today or succession tomorrow, the partnership can adapt as your goals change.

What onboarding looks like.

Once an agreement is signed, here's the shape of how a practice gets up and running with us. Each onboarding adapts to your practice. These are stages, not a fixed sequence.

01

Foundation

Data access, BAAs, and how your systems connect with ours.

02

Workflow setup

We configure intake, eligibility, scheduling, and care coordination around how your practice actually operates, not a generic playbook.

03

Team and rhythm

We train your team, connect care coordinators across the engagement, and establish the communication cadence between us.

04

Pilot and scale

We start with a small cohort to validate the model end-to-end, iterate weekly with your team, then expand as the model proves out.

The research

What therapy-first, coordinated care actually produces.

68%

Pain reduction reported in a peer-reviewed evaluation of a coordinated therapy-first program for chronic back, neck, and joint pain.

Hinge Health, JMIR 2022.

~30%

Reduction in total cost of care in a value-based pilot that defaulted to conservative-first care for back pain.

Blue Cross Blue Shield of North Carolina, 2019–2021.

$4,160

Lower one-year healthcare cost per patient who started with physical therapy for low back pain versus delayed PT.

APTA, 2020.

These figures reflect published outcomes from large coordinated programs and value-based pilots. They inform the operating model we're building. They are not claims about specific 76 Health partner practices.

Common engagement models

You pick
what fits.

Pick what works best with your goals, team, and stage of growth.

01 · Care coordination partner

Clean patient flow. Everything else stays yours.

Best for practices with operations dialed in.

  • Receive patients with physician-reviewed care plans.
  • Keep your EHR, billing, scheduling, staff, and contracts.
  • We coordinate the recovery journey around your practice.
02 · Operational support

You focus on care. We handle the rest.

Best for owners looking to offload operations.

  • Billing, credentialing, prior auth, compliance, and payer management.
  • Keep ownership and clinical autonomy.
  • Gain a dedicated operational partner behind the scenes.
03 · Full succession

Built for transitions. Ready for what's next.

Best for owners planning a transition.

  • Flexible succession and partnership structures.
  • Preserve continuity for patients and staff.
  • Move at a pace that fits your goals and timeline.

What we are and aren't.

  • We don't take over your delivering care. Your treatment approach, techniques, and clinical judgment remain yours.
  • We don't insert ourselves between you and your patient. The relationship is yours from the first visit to the last.
  • We don't lock you into one model. Our partnership structure evolves with you.
  • We don't make you change your software. Keep what you use. We coordinate around it.
  • We don't make decisions for you. Every material commitment runs through your counsel and your timeline.

The practices we're a fit for.

We're building a small, intentional set of partnerships with practices that share a few core commitments:

  • Evidence-based care. You use validated outcome measures. You modify treatment when the data says to.
  • Established and stable. Two-plus years in practice. Your team is in place.
  • In-network with major payers (or willing to be) for the lines of business you serve.
  • Open to coordination. You're willing to deliver care against a coordinated plan and to communicate progress through a shared record.

Slow and steady, on purpose.

We're partnering with a small group of practices to refine the model before we scale. If you're thinking about operational support, succession, or coordinated care delivery, we'd love to talk.

01

Direct access.

Not a sales rep. The decisions about how the partnership works will involve you.

02

Help shape it.

Design-partner practices shape the operating model: how the handoffs work, what the care plan looks like, what operational layers we handle in what order.

03

Built for fit.

This is not a “we’ll sign anyone with a license” pitch. We’re selective, and we expect you to be too.

FAQ

Common questions from practice owners.

What kinds of practices do you work with?
Primary care, speciality, physical therapy, occupational therapy, and behavioral health practices. We focus on practices treating chronic, lifestyle-driven conditions.
Are you buying my practice?
Not unless that's what you want. There are three common engagement models: care coordination, operational support, and full succession. You choose. Most practices start in care coordination or operational support and have the option to expand the relationship over time if it makes sense.
Will I lose my clinical independence?
No. Across every engagement model, your clinical judgment and treatment decisions remain yours. The MD-signed care plans we coordinate around define goals and escalation criteria; they don't dictate technique, frequency, or specific interventions.
Do I have to change my EHR, billing, or scheduling?
For care coordination partnerships, no — you keep everything. For operational support engagements, we evaluate together. Some practices keep their existing systems; others move to a shared infrastructure during the engagement. The decision is operational, not architectural, and it's made with you.
Are you exclusive? Can I keep my other referral sources?
Yes. You keep every other referral channel and patient source you have today. We don't require exclusivity from care coordination partners.
What's the time commitment for an initial conversation?
30 minutes. We use that to understand your practice and explore fit on both sides. If we're not the right partner, we'll tell you, and we'll often suggest who might be.
